Pennsylvania Licensed Health Care Manager

Key Responsibilities
- Ensure continuity and coordination of care for members with chronic conditions and complex health needs
- Develop and coordinate individualized treatment plans, monitor effectiveness, and make necessary adjustments
- Collaborate with various departments to improve services and provide accurate information to members and providers
Required Qualifications
- Master\'s degree in a human service field or a Licensed Pennsylvania RN with 6-8 years of clinical experience
- Five years of experience in clinical, care coordination, and/or case management
- Previous experience in inpatient, hospice, or home health settings is a bonus
- General knowledge of best practices in health care, especially with special needs populations
- Registered Nurse (RN) or Social Services Licensure (LSW, LCSW, LPC) required
